How It Works:

Longevity is a holistic approach to anti-aging, looking good, and feeling even better. We use the best medicine can offer to optimize hormones, balance nutrients, detox and to offer a holistic approach to your health, weight management, energy, sleep, libido, neuromuscular issues, and mental clarity.

We understand the frustration of medical expenses especially for proactive patients who want health instead of disease management. Our clinic accepts insurance for physician consultations and offers memberships for procedures and therapies which your insurance may not cover.

With the skyrocketing costs associated with most deductibles, our plans offer incredible savings and a holistic approach to health and wellness. We have 2 levels of plans and a variety of packages to help you focus on your health without cleaning out your wallet.

Membership Pricing

  • Full coverage is $199 per month with a minimum of 12 month commitment.
  • Unlimited doctor consultations
  • Weekly therapy

  • 10% off supplements and procedures

Longevity can provide super bills for you to submit to your insurance for reimbursement.

Therapy Intensive is $375 per month for 12 therapies:

  • Jumpstart detox
  • Weight loss
  • Reduce inflammation
  • Boost immunity
  • And heal
  • Great for pain management too.

Skinny shots is $125 for 6 pack.

Create a Functional Timeline

During your initial consultation, you will spend time with our staff coordinator and lay out a timeline of any symptoms or ailments you have been experiencing. We will review all prescription medications as well as vitamins and supplements that you take consistently. We will collect and document a health history, noting any significant events that may impact your overall well-being.

Physician Intake

Unlimited one-on-one visits in the office with our staff physician during regular business hours. During these visits, we will address any health concerns regarding hormone imbalances, nutritional absorption, weight loss, age optimization, and general functional medical symptoms.

Comprehensive Blood Panel

During the initial intake, we will go over any previous blood work, draw a full blood panel and conduct nutritional testing as needed. This service is offered at a discounted rate through the health membership and will vary based on the health timeline collected for each patient.

Consultations with a Nutritionist

Your nutritionist will meet with you after your exams to go over the nutrition part of your health plan and ensure you have the resources you need to fuel your body with the nutrition that it needs.

Preferred Pricing Supplement Program

Your physician will help you customize a plan to address any deficiencies and intolerances based upon your nutritional needs. As a member, you will get access to medical-grade, high-quality nutritional supplements at a discounted rates.

Health Roadmap

Our comprehensive approach with holistic and functional medicine will help you navigate your health through a roadmap for transforming your health.

Detailed Health Progress Reports

With these cutting edge tests, we can provide in-depth reports that show a window into your internal health. Reports are specific as to nutritional deficiencies, intolerances, and recommended foods to replenish areas of concern.

Advanced Testing Available

Depending on the findings and your unique circumstances, other tests may be recommended by your physician.  Longevity has access to a wide range of cutting-edge advanced testing protocols for our members at discounted prices.

Priority Scheduling

Members get priority scheduling with physicians, coaches, and therapists before new patients are accepted for anything included in the membership or other testing or consultations that are requested.

Your membership gets you access to the monthly group masterclass. Dr. Pavlovic and Shawnee Starr provide in-depth coverage of topics to help you with lifestyle transformations.
